Working with Dr. Peak

We are all digital; everything is online via your Secure, HIPAA-Compliant Patient Portal login, including your Personalized Treatment Protocol recommendations & instructions, Messages to/from your Doctor, Lab reports, Appointment requests, and Account Statement/Billing.

We do not take any Insurance or Medicare, as insurance companies like to limit treatment options to what they think is “best”, meaning cheapest, in the short term.  Your treatment plan can save many thousands of dollars over the rest of your life because reversing chronic disease can decrease future expenses, such as escalating medication needs and Long Term Care (nursing home) placement which costs over $87,000 per year in Indiana in 2021.  Your health goals are an investment in your own future.

The way that Dallas  Peak, MD works with patients differs considerably from the conventional medical model you may be accustomed to. Please read the following information thoroughly to familiarize yourself with the process. There is also a section at the end that describes the fee structure. Peak Health functions as a consultant office, not a primary care practice, and thus we do require that you maintain a relationship with a primary care provider such as an MD, DO, NP, or ND.

Dr. Dallas Peak works only with clients who live in Indiana.  In-person visits to our office located in Greenwood, IN are required for the first appointment and once per 365 days to maintain an active account with us. Follow-up appointments after the first appointment may take place in person or via phone or TeleHealth video. In-Person visits will require everyone in attendance to wear a mask during recognized periods of airborne-spread infection risk.

To become a private client, follow these steps to apply:

  1. Read the information below to understand how we work with patients and make sure you are familiar with the process and fees before you apply.  This is a generalized guide.
  2. Fill out the initial application by clicking the blue button/link at the bottom of this page.
  3. Within a week after you register in the EHR, we will contact you by secure EHR message to provide instructions for initial paperwork needs and to request your initial appointment.

We recommend that you add the domain “@icfmed.com” to your safe senders’ list in your email client to make sure that our emails do not end up in your spam folder.  Email is the main communication tool we use for your Portal notifications.

For an IntellxxDNA ™ Genomics Consult experience, your package has three appointments:

  • An Initial, brief appointment to choose your package, order your saliva test kit, and fill out forms.
  • A 90-minute appointment to review the most important findings, followed by a 60-minute appointment within 60 days, to reveal the rest of your genomic insights.

As a new Functional Medicine client, your treatment is broken down into two phases:

  • The Case Review (Phase One), includes 2 appointments
  • Ongoing Treatment (Phase Two), the follow-up appointments

Each phase is described in more detail below:

PHASE ONE: The Case Review

The Case Review Process is the first step for all new clients. The Case Review process is designed to save money and time in the long-term by performing the appropriate diagnostic testing and evaluation before treatment begins. A cornerstone of our approach is “test, don’t guess.” Identifying the underlying patterns contributing to disease is the key to a successful and lasting outcome.

The Case Review process serves several purposes. It enables Dr. Dallas and/or Dr. Pamela to more accurately estimate how long your treatment program will last, what it will include and estimate how much it may cost. It gives you the chance to ask any questions you may have about your condition and what you should expect. It also provides an opportunity for us to get to know each other and determine if our clinic is a good fit for you over the long-term.

The Case Review process includes:

  • An initial 2-hour consultation to determine which tests to order. (The “Initial Consult” appointment).  
  • A thorough review of your medical history and relevant previous lab work (20 page limit).
  • A review of your current diet and nutritional supplements.
  • A detailed body systems assessment and  a physical examination by Dr.Peak
  • A 3-hour in-person consultation with Dr. Peak, to review your test results and present your treatment plan. (The “Case Review Consultation” appointment)
  • Treatment plan includes diet, supplements and lifestyle therapies.
  • Recommendations for further testing as indicated and planning for the next appointment.
  • Referral to a Health Coach &/or Dietician to assist you with your health goals.

The Phase One Case Review process includes two steps, which are described in more detail below.

 

Phase One, Case Review, Step One: Initial Consult

During this 2-hour appointment, Dr. Peak will interview you to determine which lab tests to order for your Case Review based on your current concerns and your history and initial intake questionnaires.

This appointment has two purposes: to make sure that we have all of the information needed to gain a comprehensive understanding of many of the factors affecting your health and to give you a head start on your treatment

The exact lab testing ordered at the Initial Consult depends on your individual circumstances, but may include:

  • A comprehensive blood chemistry panel. This is the single most efficient, effective and affordable tool for quickly evaluating your health. It screens for a wide range of conditions, including several types of anemia; gut, viral and bacterial infections; insulin resistance and hypoglycemia; liver and kidney issues; and thyroid and adrenal problems. It offers important clues for how to structure and focus your treatment to get the best results. It also provides a baseline of biomarkers that can be used to objectively track the progress of your treatment over time.
  • Additional blood tests for specific conditions, such as high cholesterol, hypothyroidism, autoimmune disease, osteoporosis, chronic infections & biotoxin illnesses, heavy metals, environmental toxin exposure, gluten/food sensitivities, & genetic testing.
  • Advanced stool testing to screen for parasites, fungal overgrowth, bacterial infections, intestinal inflammation, intestinal permeability, dysbiosis or a deficiency of beneficial bacteria, metabolic products of bacteria, and inadequate digestion of nutrients.  A 3-gas SIBO Breath Test may be needed too.
  • Urine organic acids testing to screen for small intestinal bacterial overgrowth, fungal overgrowth, problems metabolizing fat or carbohydrates, B-vitamin status, methylation issues, detoxification capacity, oxidative stress and neurotransmitter metabolism.
  • Urine/saliva hormone testing for adrenal and sex hormone status and their metabolism.
  • Cognitive Testing at initial/baseline and repeated during follow-up.
  • Genomic evaluation to uncover personal risks and health traits.

You will be provided with several electronic health history forms to complete within the Client Portal of our Electronic Health Record (EHR), before your first appointment. It is important that you spend time and energy necessary to complete these intake forms, as they are vital in helping us understand your case. These forms can include:

  • A detailed health and medical history questionnaire
  • A survey of your chief concerns and most important health goals
  • An assessment of your most troubling and frequently experienced symptoms
  • A diet survey and questionnaire
  • A survey of your current supplements and medications

Finally, we will provide you with dietary guidelines to follow after your Step One Initial Consultation. Dietary guidelines are tailored based on individual circumstances, such as for the ReCODE ™ Protocol (using Dr. Bredesen’s KetoFlex 12/3 Diet), Mediterranean, or other special physiologic needs or restrictions such as low-FODMAP or autoimmune elimination food plan.

It is critical that you implement the suggested advancements in your diet to the best of your ability. When you speak with Dr. Peak at the next appointment, your experience with the diet will be as important as any lab findings in guiding the next steps in your care plan.

Once we’ve received your lab results and complete Case Review paperwork, you will move on to Step 2: the Case Review Consultation. This typically occurs between 30 and 60 days after the Initial Consultation, because some of the specialty labs we use take up to 4 weeks to deliver the results after receiving your sample. While you wait for your lab results to come back, you will be implementing your new diet and making forward progress.  This is also the timeframe that you will be engaging with your health coach for assistance with implementation of your dietary advancements and getting things “done”.  We find that a dedicated Health Coach is a key part of improving your health toward your goals.  Health Coaches have “been there, done that” with many health journeys and can share a wealth of experiences and assist you in finding ways to engage your new changes and any recommendations needed for the next appointment, as well as helping with accountability if needed.  Peak Health does expect the client to engage with a Health Coach in-between our first 4 appointments, at a minimum.  This is at an added cost, that varies with the Health Coach, and helps clients fully benefit from their custom plan from Peak Health.

 

Phase One, Case Review, Step Two: In-Person Consultation

The Case Review Consultation is a 3-hour in-person appointment. If you live outside the Central Indiana area, you must make arrangements to travel to our clinic for the appointment.

Prior to the Case Review, Dr. Peak will review the results from the labs that were ordered along with your Case Review paperwork, medical history, diet and supplement survey, any additional assessment forms, and relevant prior lab work. Then they will create a Report of Findings, which contains three parts:

  • A summary of the underlying patterns that are contributing to your symptoms.
  • An outline of the treatment plan, including dietary, supplement and lifestyle recommendations.
  • Recommendations for further testing, as indicated by findings so far.

During your Case Review Consultation, Dr. Peak will present the Report of Findings as well as your treatment plan. They will also review your test results with you, discuss the results of your initial food plan, and answer any questions you have about the findings or the treatment plan. If applicable, ReCODE/PreCODE ™  Reports will be provided/discussed.

 

PHASE TWO: Ongoing Treatment

Once the Case Review is complete, you’ll have follow-up appointments on an as-indicated basis throughout the remainder of your treatment plan.

The first follow-up appointment generally occurs between six and 12 weeks after your Case Review Consultation. At that time, Dr. Peak will review your progress on the treatment plan, along with any follow-up test results, and make additional recommendations as necessary.

From there, the frequency of your follow-up appointments with Dr.Peak will depend upon your anticipated treatment plan, and the severity of your condition as well as your response to the treatment. ReCODE ™ and PreCODE ™ followup appointments specifically are every 3 months for the 1st year, and at least every 6 months thereafter.

You may schedule follow-up appointments in person in our office, or via phone or TeleHealth. We ask that you return for an in-person visit at least once per year to maintain an active account.  In some circumstances, follow-up communication can be done within your Portal at regular hourly rates.  There is no charge for quick, yes/no answers or clarifications.  We want you to understand your program!  There is charge for responding to items that require us opening up your chart, such as: messages submitted to us via the Portal messaging system that involve Medical Decision Making, adjustments to protocols, prescription refills/changes, time in research or filling out forms on your behalf, communication with other practitioners, and data entry functions that may be required in special programs such as the Bredesen ReCODE ™ Protocol.   An exhaustive list of possible charges is not possible in this writing.

 

 

FEES & COSTS:

Dr.  Peak

(the most current cost estimates are published on this page, and necessarily cannot cover all possibilities)

Initial Consult: $329. Includes a 2-hour appointment,  Initial Dietary Guidelines, and Case preparation time.

Case Review: $1195. Includes the 3-hour in-person consultation with Dr. Peak, and the Report of Findings (& ReCODE Report if applicable) with treatment program instructions based on the interim testing results.

Regular Follow-ups: $240 for 30 minutes, $340 for 45 minutes, and $400 for 60 minutes. The hourly rate for all other tasks is $400/hour.

Portal Messaging (Advice/Medical Decision Making), Back-End work, ReCODE ™ Data Entry, and Tasks requiring opening/reviewing your chart performed on the behalf of the patient:  Billed in 10-minute increments, at $66 for each 10 minutes or each part of 10 minutes [the Hourly Rate].

Rx refills outside of an appointment   $25 for time to review your records, contact pharmacy, and document in your chart.

Generating Medical Records to send to another practitioner or office: You can download & print your records for free in your Portal at any time.  Up to 10 pages of your records can be officially faxed by your doctor without charge.

Beyond 10 pages, Physical paper records will be created at our Hourly rate (for printing and transporting to post office), plus Printing Costs (10 cents per page), Postage and Driving time. Alternatively, you may download/print ALL of your records for free in your Portal and share them as needed.  As we do not work with insurance companies, if they want medical records, you have the choice to provide your records to them for free from your Portal, or via the charges from Dr. Peak.

Disability: Dr. Peak does NOT fill out medical disability forms.  On very rare occasion, Dr. Peak may write a letter to detail medical necessity for something.  Under such circumstances Dr. Peak bills at the hourly rate to write such letters.  Dr. Peak does not submit medical notes to support disability claims.

 

Laboratory Tests

The investment for laboratory tests ordered during the case review process is typically $3500-$6000 (if not using any insurance to pay for labs), depending on complexity of condition and level of diagnostics required. The cost of your testing is generally between you and the lab. The cost of rare testing may include an administrative and/or interpretation fee if purchased directly from Peak Health.  This is because a few laboratories will not take payment from patients or insurers, thus requiring us to collect the money for these tests.

Imaging Tests

The cost for imaging studies, such as those recommended for the ReCODE ™ Protocol evaluation, is in addition to the above, and your insurance may help pay for those costs.

ReCODE ™ (& PreCODE ™) Protocol specific enrollment-associated costs

ReCODE ™/PreCODE ™ Protocol patients must enroll in the ReCODE ™ Program at Dr. Bredesen’s program’s administrative website.  The ReCODE Report ™ is exclusively available for members.  Information is online at Dr. Bredesen’s program administration website.  The steps are (1) enroll, (2) Order and receive Bloodwork (lab) instructions, (3) Complete their Medical Questions and Cognitive Assessments, (4) Generate your first personalized ReCODE Report ™, (5) Locate and choose your trained Recode ™ Practitioner, (6) Review your Report with Qualified ReCODE Report ™ Practitioner, (7) Start the Bredesen Protocol ™.  Dr. Bredesen’s team at Apollohealthco is a separate entity, not directly connected with any practitioner’s offices.  If you have questions about ReCODE ™ (or PreCODE ™), please contact them directly at 1-800-450-0805 or email  info@ahnphealth.com; Dr. Bredesen’s  website is  www.apollohealthco.com/

Supplements & Therapies

The cost for supplements that are advised as part of your treatment plan after the Case Review is typically $200-$650 per month. You will have the opportunity review the supplements Dr.’s Peak suggest before you purchase them (after the Case Review Consultation). Some supplements are only sold through practitioners’ offices; most are available from trusted online distributors that we recommend (such as FullScript.com). Peak Health does not normally stock supplements but is able to obtain items for you that can only be sold through a practitioner office.  As well, if you request, we can source your supplements and you can pick them up directly from the office, at full MSRP.  We do provide a significant discount for our clients of record at trusted, online vendors such as FullScript.com.

The cost for any Therapeutic Modalities, Dietitian Services, Health Coaching, or Exercise Programs is in addition to costs from Peak Health, as they are not invoiced by us directly.  Peak Health expects our clients to work with a Health Coach &/or Registered Dietitian.

 

Grand Total of Case Review Fees

The total you could expect to pay for the entire Case Review process with Dr.’s Peak is roughly $3500 -$7500 plus any imaging, plus your Health Coach/Dietitian fees, if paying by cash.  Many of the lab test and imaging companies will submit invoices/superbills on your behalf to your insurance, however.  We are unable to estimate how much your policy will cover and advise clients to contact their carriers directly about lab & test coverage.

 

Estimated Costs for Laboratory Tests for Ongoing Treatment

Lab test costs going forward vary considerably depending on followup needs and treatment plan, but could average approximately $200-$500/month, if needed.

IntellxxDNA Genomics Package:

Privacy/HIPAA information:

Peak Health LLC/ICFM is not bound by the traditional privacy requirements in the Health Insurance Portability and Accountability Act (HIPAA) because this office only accepts self-payment. However, Peak Health LLC/ICFM is dedicated to providing reasonable standards of privacy and confidentiality of each patient’s health records.

Peak Health LLC/ICFM is an office which may provide training to clinicians and administrative staff who may or may not be employed by Peak Health LLC/ICFM.  In the course of training, the clinicians and/or administrative staff may have limited or full access to a patient’s health record, for purposes that may or may not involve your care.  All trainees are required to sign a confidentiality agreement to protect the privacy of your information consistent with our Privacy Policy.
